ACCIDENT MEDICAL/DENTAL EXPENSE PROGRAM A FULLY insured, ANY provider - Worldwide reimbursement plan. Available Nationwide, Guaranteed Issue (you cannot be turned down) This Insurance is Great for people that are active and do activities where accidents can happen (i.e. hiking, tennis, football, hunters, snowmobilers, biking and any others). Also for people that have NO coverage at all. Issue Ages: 18 to age 72, then renewable to age 75. Plan can cover: Single member, lawful spouse and dependent children under age 19 (or to age 25 if a full-time student). Worldwide coverage, 24 hour Accident Medical/Dental protection. Incident deductible $100.00 then benefits paid at 100% UCR to chosen max. Choice of (3) plan maximums, $10,000, $7,500 and $5,000 "per" incident. Accidental Death & Dismemberment benefits included as follows: $10,000 Plan ($50,000); $7,500 Plan ($25,000) & $5,000 Plan ($10,000) Insurance Payouts: 100% of the face amount for the loss of life, both hands, both feet 100% of the face amount for the loss of entire sight of both eyes, one hand and one foot 50% of the face amount for the loss of one hand (or one foot) & entire sight of one eye 50% of the face amount for the loss of speech or hearing in both ears 25% of the face amount for the loss of thumb and index finger of same hand Accidental Medical/Dental Incurred Expenses are as a result of injury, an insured incurs covered expenses starting within 30 days from the date of the accident causing the injury, this plan will pay 100% (less deductible of $100.00) and not to exceed the chosen maximum benefit. All covered expenses within one year from such date. Covered expenses mean the reasonable & customary charges for local professional ambulance service to or from a hospital and/or surgical center as well as the following reasonable & customary charges for treatment, services & supplies provided or prescribed by a Doctor. Charges include: X-rays, lab tests, oxygen, casts, splints, crutches, braces, blood, blood plasma, drugs, medicine & treatment of natural teeth, durable medical equipment, etc. |
